Resources to Address Social Determinants of Health
What are Social Determinants of Health (SDoH)?
These are non-medical factors that affect health and well being and may include conditions in which people are born, grow, work, live, play and pray. Is your medicine too expensive? Do you have a doctor? Do you have a way to get to your appointment? Are you getting your health information in a way you can understand? These are a few examples of the effects of SDoH.

Identifying Mental Health Needs for Faith Leaders

As you know, faith institutions are more than places of worship, they are vibrant communities with shared visions and a commitment to build a bridge between faith and wellness. Because we have a community partnership or connection with you, we are asking you to please join Texas Health Community Health Ministry to empower faith communities in addressing mental health concerns among congregants and work alongside of us to build that bridge. We are asking you to provide us with feedback on what you believe to be the most critical issues on mental health among your congregants.
This request for information is based on Texas Health taking a deeper dive into results from previous surveys you participated in. All information is confidential and used only to provide resources/education materials to you. Thank you and we look forward to hearing from you!
Maternal Health

Texas Health Resources recognizes the role we can play in proactively and holistically addressing poor health outcomes in many of the communities we serve. We understand that empowering and ensuring access to maternal healthcare is key to addressing challenges and improving outcomes for all moms and babies.
Research has shown that faith-based health promotion interventions can have a significant impact on health behaviors and outcomes. These programs often incorporate spiritual and cultural perspective, making them effective and beneficial for diverse groups. There are too many women dying from pregnancy complications in the U.S. The Center for Disease Control and Prevention (2024) has reported that more than 80 percent of pregnancy-related deaths are preventable.
